There is no specific format. Feel free to lay it out as you like, just make sure it's understandable.

I agree it would be easier for the person doing the request if he/she had could issue a layour that the requester should use.

Depends. Some people would be happy to make something for free.

But others ( like me in some cases) would prefer to be payed.
