HowTO – add text to pdf file – MDH forms

Problem – I need to add text data to MDH pdf form and then return form to MDH.
Solution – Open MDH pdf 2 page form in Gimp > file > open > MDH.pdf > import > select all (two pages) > open pages as IMAGES > import > tools > text > font sans, 16.3 pt > draw text boxes >type information in text boxes > add check marks on form "X"
Now to save to pdf file format -
File > print > print to file > name - “hospital2.pdf” > save in folder - MDH > output format - pdf > print
This was done in PCLinuxOS 2011.
